To apply for registration for the Workshop, click
on the .PDF fill link listed below. Once you have opened it,
fill it out and hit the "print form" button. This will automatically
bring up your printing agent. Then hit the "print" button
in your email agent. Then fax it to us at 318/356-9119.

Applications will be accepted through April 2, 2004.
Participants will be notified no later than April 9, 2004. Fees
will be due after notification of acceptance to the workshop Fees:
$550.00 payable by check and
money order
(make payable
to NSU / NCPTT CMC 2004).
